Add more information about Bluetooth usage.
The patch shows how to create a remote device connection and
how to manage devices in /etc/bluetooth/hosts. The multimedia
chapter was extended with a description on how to set up and
connect a Bluetooth audio device as a sound sink.
It also adds man page references to man-refs.ent for hcsecd(8)
and virtual_oss(8), respectively.
I cleaned up the patch a bit for consistency and proper markup.

Prefer the use of service(1) to explicit invocation of /etc/rc.d
scripts: service(1) puts things in one place and abstracts them. Also, it's less
typing and easier to autocomplete.
While here modernize script examples by removing the ".sh" suffix.

Merge the projects/sgml2xml branch:
- Documentation is updated from DocBook 4.1/SGML to DocBook 4.2/XML
- Webpages are updated from HTML 4.01 Transitional to XHTML 1.0 Transitional
- Static webpages are now processed by XSLT behind the scenes
- Webpages are now built with less cycles; tidy has been removed and the
date processing is now done by XSLT
- Generated webpages are now actually valid (they did not use to be)
- All XSLT stylesheets now pull in a main XSLT, which reduces
duplicated markup
- Site map and index are converted to an XML format with an
XSLT transformation that generates the output
- For docs, there is now only one entity set for both articles and books
- Some trademark/legalnotice entities have been merged to a cohesive
single entity file
- Untranslated entity sets are now always pulled in from the English tree
instead of redundant copies
- The base and enbase entities are already automatically generated so
remove inline definitions from individual files
- Fetch the LEGAL file via http instead of depending on CVS
- Convert id names to lowercase to avoid mixing different styles and
for better readability
- All PSGML comments are removed since they are mostly useless
- Convert character entities to normal characters
- Do not copy CSS files but override only necessary part
- Fix build with newest tidy
- Partly update the fdp-primer to reflect changes

This change adds a new section about MythTV to the multimedia chapter
of the handbook. The section is based on notes from a page in the
FreeBSD wiki.
Converting the wiki page into a handbook chapter was done as part of
the Google Code-In 2011 contest.

Add a description and workaround for the problem where the built-in
sound devices of graphic cards are being assigned lower device numbers.
In order to use them, one has to change sysctls to the proper device
number than is being shown in the dmesg output.

Do what I swore would be done:
o Remove 4.X information, this includes notes and sections.
o Update documentation to reflect 5.X and 6.X.
o In some areas, try to make the new content version agnostic.
o Skip areas stating "5.[0-5] and later" as it's relevant - we

For several items, I checked the NOTES files, manual pages,
CVS history, etc.

In scanners section:
By default, under 5.X, uscanner0 permissions are 0640 preventing users
in operator group to use the scanner (a write access is required). So
let's show how to change the permissions to 660 (for example) via
devfs.rules and the devfs_system_ruleset option in rc.conf.
This gives an example on how to change permissions on a node that is not
always present at system boot.
This form allows you to request diffs between any two revisions of this file.
For each of the two "sides" of the diff,
enter a numeric revision.
